YOUNIQUE YOGA A DBA of Firelily Holistic Healing LLC Assumption of Risk and Waiver of Liability By attending any class, workshop, or event at Younique Yoga, a DBA of Firelily Holistic Healing LLC, each participant acknowledges and agrees to the following terms and conditions: 1. General Assumption of Risk The participant voluntarily assumes all risks associated with participation and hereby waives any and all rights to pursue legal action, claims, or proceedings — including jury trials — against Younique Yoga, Firelily Holistic Healing LLC, its owners, instructors, and staff, arising out of or related to participation in any class, workshop, or event offered by Younique Yoga. 2. Communicable Disease / COVID-19 Assumption of Risk The participant acknowledges that attendance at Younique Yoga may involve potential exposure to communicable diseases, including but not limited to COVID-19 and its variants. The participant voluntarily assumes all risks related to such exposure and agrees that Younique Yoga, Firelily Holistic Healing LLC, its owners, instructors, and staff shall not be held liable for any illness, injury, or damages resulting from exposure to any communicable disease during attendance at any class, workshop, or event. 3. Health Acknowledgment The participant confirms that they are not experiencing any symptoms of illness, have not recently been diagnosed with COVID-19 or any other communicable disease, and are attending voluntarily and at their own risk. 4. Indemnification The participant agrees to indemnify and hold harmless Younique Yoga, Firelily Holistic Healing LLC, its owners, instructors, and staff from any and all claims, damages, or expenses arising from their participation in any class, workshop, or event. 5. Package Expiration Policy All class packages purchased at Younique Yoga expire six (6) months from the date of purchase. Any unused classes or sessions remaining after the expiration date will be forfeited and are non-refundable. By signing this waiver, the participant acknowledges that they have read, understood, and agree to this policy.
