I hereby agree to all of the following: I/my child am/is participating in the Dance and/or Wellness Classes offered by Impact Dance x Wellness. I recognize that these classes require physical exertion, which may be strenuous and may cause or aggravate physical injury, and I am fully aware of and willingly assume the risk and hazards involved. I understand that it is my sole responsibility to consult with a physician prior to and regarding my/my child’s participation in any of the offered classes. I represent and warrant that I/my child am/is physically fit and have /has no medical condition which would prevent full participation in the classes. I agree to assume full responsibility for any risk, injuries, or damages, known or unknown, which I/my child might incur as a result of participation in the program. I knowingly, voluntarily and expressly waive, renounce and disclaim any claim I have or may have against Impact Dance x Wellness, its directors Sara Gonzalez/Meagan Manahan, instructors/guest instructors, and employees for injuries or damages that I/my child may sustain as a result of participating in the program. For myself, my heirs, successors and legal representatives I forever release, waive, discharge, and covenant not to sue Impact Dance x Wellness for any injury or death caused by their negligence or other acts that are not intended to harm me/my child. I will not hold responsible/sue Impact Dance x Wellness. Releases for any injuries suffered by me/my child caused in whole or in any part by my/their failure or refusal to follow the instructions of you or your instructors or by any physical impairment of mine/theirs. All fees paid hereafter are non-refundable as a matter of right; such refunds if any, as are made shall be entirely within the sole discretion of Impact Dance x Wellness. I understand that there are no crediting of any payments, and that items purchased from the boutique may be exchanged only if they have not been worn, damaged, or washed. I acknowledge a $15 fee for late payments ($20 for dance competition payments), and a $35 fee for returned checks. I understand the inability for me/my child to participate in events if payments have not been received by the due date. If a student discontinues enrollment at Impact Dance x Wellness with an outstanding balance, that balance must be paid in full within 30 days. If not received after 30 days, the balance will be sent to “Legal Recovery.” I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above and acknowledge that Impact Dance x Wellness would not agree to permit me/my child to participate in any of its programs if I/my child chose not to. I acknowledge that Impact Dance x Wellness reserves the right to refuse service to anyone. By signing below, I agree to have read and understood the above disclaimer and acknowledge that more information is available to me in-studio and on the Impact Dance x Wellness website.
